Fly fishing set the hook?

Ok, so lately I've been fishing this lake (trout fishing) and almost Whenever I catch a fish always seem to lose the fish I am about to network. What I'm thinking how I'm hooking Fish? If i jerk the rod when I get a bite or I draw the line as fast as I can not break the bar even higher? Always keep tension when the fish is my fight, even if an end to a float tube, but the fish that manage to come off. What am I doing wrong?
